Anyway, a lot as been asking about the hospital lists that I made for my delivery so here they are! As some of you might know, I am quite OC about paperwork and I have no pain tolerance so I really wanted to make sure that my doctors did not miss anything about my medical history. I was also confined 3x during my pregnancy. This is why I wanted to make sure that my list is detailed so we could avoid mistakes.
This is the information sheet that I made. I consulted my OB prior to my delivery about the things that will be asked during check in/ labor. The text is
RED are things you need to fill out on the way to the hospital as you might not have enough time to print them when you start contracting.

My simplified birth plan! Basically, I just want to make sure that everybody knew that I have ZERO pain tolerance. đđđ My plan was actually followed! I hated getting my IV inserted because it was so painful. The resident anesthesiologist injected me with a light anesthesia using a baby needle just so I don't feel the IV going in. Amazing! I also wanted my baby to be monitored continuously as I was being induced. I had a very sensitive pregnancy. I had to make sure that little E is healthy all throughout the process. My advice to first time moms is to relax and be open minded. Trust your doctor. After all, he/she took care of you and your baby up to this point.

As a bonus, fill out this Birth Certificate cheat sheet as well so your husband can fill this out for you even when you are still in recovery. It's such a hassle to make a mistake and to have to file for an affidavit of correction afterwards. Fill this up ahead of time to save you the trouble!
